VEGETATION SPRAYING AT FACILITIES FORM (SMO4A)
This form will be presented by Knox County School employees before the use of vegetation control chemicals at a facility.
The purpose of this form is to inform the administrative staff of the intent to use potentially hazardous chemicals and to ensure that Material Safety Data Sheets are available for these materials.
This form should be signed by a member of the administrative staff of the facility so that proper authorization is obtained.
